Prepare for your job shadowing application by reviewing the requirements and resources below.
Requirements for Shadowing
For Shadowing 12 Hours or Less
- PPD (TB Skin Test) or IGRA (QuantiFERON Gold or T Spot) from the past 12 months
- COVID Vaccine Card/Documentation or EAH COVID Declination Form
- Flu Vaccine Documentation for Current Season or EAH Flu Declination Form
For Shadowing More Than 12 Hours
- All the above, plus:
- Documentation of all 3 HEP B vaccines or positive HEP B titer
- Documentation of both MMR vaccines or positive MMR titer
- Documentation of 2 Varicella vaccines or positive Varicella titer
- Documentation of recent TDAP (within 10 years)
